Dental Hygiene Licensure by Credentials Print E-mail


Important - Do you hold a Canceled Texas Dental Hygiene License?  
If the answer is "Yes" do NOT apply for a new license using the Licensure by Credentials application.  A dental hygienist who has been licensed in Texas, moved to another state, is currently licensed and has been practicing dental hygiene in the other state for the last two years, may obtain a new dental hygiene license without reexamination.  Please click HERE to read more on reinstating a canceled dental hygiene license or contact the Texas Dental Board at (512) 463-6400 for more information.

Disciplinary Action:
Applicants for Licensure by Credentials must not have been the subject of a final disciplinary action and is not the subject of a pending disciplinary action in any jurisdiction in which the dentist or dental hygienist is or has been licensed.

Application for Licensure by Credentials

Dental Hygienists applying for licensure by credentials will be required to obtain a Level II background information report from Professional Background Information Service (PBIS).  PBIS charges a fee for their service and more information is available on their website. The applicant will apply and pay fees directly to PBIS to initiate this service.  For appropriate PBIS application information, contact:

Documentation for PBIS

Note:  (Applicants with final or pending disciplinary action are not eligible for licensure by credentials).

The following is a list of documentation that will be collected and verified by PBIS prior to your application to the Texas State Board of Dental Examiners for licensure:

-  Birth record or verification of identity.
-  Verification of graduation from an ADA-accredited dental hygiene school.
-  ADA National Boards, Parts I and II.
-  Verification of a dental hygiene clinical examination (state or regional)
-  Verification of licensure in all states where licensure is/was held.
-  Current CPR certification in basic life support.
-  Verification of dental hygiene practice for at least 3 of the 5 years preceding your application to Texas; or verification of your employment as a dental hygiene educator for five years preceding your application to Texas.
-  Two letters of character reference (author signature must be notarized)

Application Processing Time and Contact with PBIS

Please note that it may take 45 to 90 days for PBIS to complete this process. You must contact PBIS directly for the status of your application.

You will be notified in writing by PBIS when your complete package is sent to the Texas State Board of Dental Examiners. When you receive the written notification, you may complete an Application for Licensure by Credentials for Texas.   Please attach a copy of the following documentation with your application for licensure:

-  The letter from PBIS verifying that your packet has been submitted to the offices of the SBDE.
-  A copy of the notification of successful completion of the Texas jurisprudence exam.
-  Proof of 12 hours of continuing education taken within the twelve months preceding your application to Texas.

-  Appropriate application fee. (Personal checks, cashiers checks, or money orders are accepted)

Your application will be processed and a license number issued within 2 to 4 weeks.
