Dental Hygiene Licensure by Examination

Fingerprint Criminal Records Check Requirement When Applying for Licensure by Examination

Jurisprudence Requirement for All Licensure Applicants

All applicants for licensure in the State of Texas must complete the SBDE Jurisprudence Assessment for Initial Licensure online.  To learn more about this process and take the SBDE Jurisprudence Assessment for Initial Licensure click here:  www.tsbde.texas.gov/Jurisprudence   


Application Process

Applicants educated in a school recognized by the Commission on Dental Accreditation of the American Dental Association and completing a regional clinical exam as outlined in #6 below, should include the following documents.  Photocopies are acceptable, except where otherwise indicated.

1.   Complete and notarized Dental Hygiene Licensure Application.  (A complete checklist of all items and requirements for licensure is available on the application).

2.   Application fee of $120 (no cash please).  Checks/money orders should be made payable to the State Board of Dental Examiners (SBDE).

3.   Xerox copy of official birth certificate, naturalization papers, or passport that indicates date of birth.  Hospital birth record not acceptable.

4.   A certified or notarized copy of diploma or a letter of degree award from a dental hygiene program that must be signed by the Dean or Registrar.  All institutions must be recognized by the Commission on Dental Accreditation of the American Dental Association.  In lieu of sending a diploma, we will accept a letter issued by the Dean's Office or Registrar's Office that clearly states you have graduated from the dental program
 
5.   Proof of successful completion of National Board Examination    (A certified or notarized copy of computer scorecard or certificate).

6.   Proof of successful completion of ONE of the following:  

(Examination results will be accepted for five years from the date of the examination.)

      a)   Western Regional Examining Board   - www.wreb.org   (602) 944-3315
             -  Exam Results must be dated within the last five (5) years and:
                  --  Exam Results dated prior to January 1, 2010 must be certified or notarized.
                  --  Exam Results dated after January 1, 2010 do not need to be certified or notarized.

      b)   Central Regional Dental Testing Service  - www.crdts.org   (785) 273-0380
             -  Exam Results must be dated within the last five (5) years and:
             -  Exam Results must be certified or notarized.    

      c)   North East Regional Board  -  www.nerb.org   (301) 563-3300
             -  Exam Results must be dated within the last five (5) years and:
             -  Exam Results must be certified or notarized. 

      d)   Southern Regional Testing Agency   - www.srta.org   (757) 318-9082
             -  Exam Results must be dated within the last five (5) years and:
             -  Exam Results must be certified or notarized.  

      e)  Council of Interstate Testing Agencieswww.citaexam.com   (919) 460-7750  
            -  Exam Results must be dated after January 1, 2009 and:
            -  Exam Results must be certified or notarized. 

7.   If you are licensed in another state or jurisdiction, verification/certification of licensure with imprint of state seal issued to you by the dental hygiene board in each state in which licensure is held.  This document must contain a statement indicating if disciplinary action has/has not been taken against the license.  A copy of the state license is not acceptable.  Contact the issuing agency for fees and procedures associated with obtaining the document.

8.   CPR Requirements.  Photocopy of current (as indicated on your card) basic life support CPR certification.  Note:  CPR training must include a written assessment and a hands-on demonstration of skills by course participants.  Computer and video-based CPR training courses given by an instructor that include a hands-on demonstration of skills by course participants on a practice manikin and include a written assessment of skills are acceptable.  Online courses that do not include a hands-on demonstration of skills or a written assessment are not acceptable.

9.   Proof of successful completion of SBDE Jurisprudence Assessment for Initial Licensure dated within the preceding twelve months. 

10.  Proof of Social Security Number.  (Photocopy of card acceptable)

11.  Xerox copy of receipt issued by L1 Enrollment Services showing proof of having your fingerprints collected for the required fingerprint criminal records check. Information on this process is available at: www.tsbde.texas.gov/Fingerprint


Application Receipt and Processing Time

It is strongly recommended that you submit your package via certified, return receipt postage to allow you to track delivery thorough the carrier.  Please check with the delivery carrier before contacting the SBDE to verify shipment and receipt of your application package.

It will take approximately six to eight weeks to process your application for licensure.  During graduation season (May to July) average processing time may take longer.
